Chronic Disease Management

chronic obstructive pulmonary disease or hypertension the practice
offers specialised clinics to advice on healthy living and best
treatment to maintain your symptoms. |
Aural Care
 | Specialist nurses will syringe ears, treat
and advice on ear problems. |
Breath Test
 | A simple test to detect if your stomach is infected
by a particular germ. |
Cervical Screening
 | Smears should be carried out every 3-5 years. If you
are due a cervical smear, please speak to one of the reception team. |
CVD Risk Assessments
E.C.G.
 | A test to measure activity within the heart muscle. |
Extended Operations
 | The practice also provides complex procedures such
as Vasectomies, removal of ganglions, varicose veins. |
Family Planning
 | Specialist nurses and GPs can offer advice on the
best contraception method for you, this could include the pill, coil
fitting or contraceptive implants. |
Lung Function
 | A special test designed to diagnose chronic
bronchitis or emphysema. |
Minor Operations
 | The practice provides minor surgical procedures
under local anaesthetic. These include removal of skin-tags, small
cysts and joint injections. |
Non-NHS Examinations
 | If a medical examination is required for special
purposes such as taxi, pre-employment, Heavy Goods Vehicle licence,
diving, insurance, etc. Please make a separate appointment for which the
appropriate fee will be payable. |
Phlebotomy
 | Health care technicians and trained phlebotomist will take blood for testing at the hospital. |
Smoking Cessation
 | Support and advice on treatment to help you stop
smoking will be given by a specialist nurse. |
Travel Clinic
 | The nurse will offer advice and plan your
immunisation programme for your chosen destination. |
Weight Management Clinic
 | A specialised clinic to maintain and help manage
weight problems. |
Well Man Clinics
The health visitors attached to the surgery offer a variety of services:
Well Baby Clinic: Held every Wednesday (13:30 - 15:00 appointment only). This clinic offers the opportunity for you to bring your baby for weight/growth checks and discusses
parenting issues with the health visitor. This clinic is for well babies only – if your child is unwell they need an appointment with a GP.
Child Health Assessment: This involves regular assessment of children up to the age of five years old by the health visitor and GP.
