Car Park

A large car park is provided at Ash Grove for patient use. Please use the car park at the front of the building. Car park at rear of the building is for Staff only. A bike park is provided at the rear of the building.
(The practice cannot take responsibility for patient's cars you park at your own risk).

Complaints & Suggestions

We aim to provide a high class service. If you have any suggestions on how we can improve our service then please address them to Mr C Ealham and leave in the suggestion box in the waiting room. If you have a complaint, please put your details in writing to Mr M Hart. All complaints will be investigated thoroughly. We have a practice based complaints procedure which we feel sure will resolve almost all problems. If we cannot resolve your complaint you may discuss the matter with a Conciliator or it can be referred to the Ombudsman.

Facilities for Disabled Patients

The building is fully accessible for disabled people and includes suitable toilet facilities, disabled parking, and ramps for wheelchair access.

This combined publication scheme is a complete guide to the information routinely published by Ash Grove Surgery. It is a description of the information about General Practitioners and the practice which we make publicly available. We shall review the scheme at regular intervals and monitor how it is operating.

Identifying Carers

Definition – A carer is someone who voluntarily looks after a friend, relative or physically/mentally disabled child who needs support to live at home.

As a carer, you are entitled to a Carers assessment by the Social Services, to discuss the help that both you and the person you care for, might need.

If you are a carer please speak to one of our reception team and the practice can start offering support and help for the both of you.

NHS Summary Care Records

The practice is now participating in the Summary Care Record scheme. This is a new development which will affect the way your health records are managed and will enable clinicians to access important health information to ensure you receive the best treatment. Please refer to the enclosed leaflet for further information.

What is a summary care record?
The NHS summary care record is an electronic record of important information about your health. It will include details about your current prescriptions, any allergies and any bad reactions to medicines you have had. Your doctor may also decide to add other useful information to it.

Why is a summary care record important?
Your summary care record will improve the safety and quality of your NHS care. It will give healthcare staff faster, easier access to reliable information about you to help with your treatment.

Who will be able to see my health information?
Your summary care record will be available to authorised healthcare staff providing your NHS care anywhere in England, but they will ask your permission before they use it. This means that if you have an accident or become ill, the doctors treating you will have immediate access to important information about your health.

Contact our local Patient Advice and Liaison Service (PALS) by telephoning 0845 6024832. Advisers are available Monday to Friday from 10.00am to 4.00pm. You can also email the service on pals@wdpct.nhs.uk with your enquiry and an adviser will get back to you as soon as possible. Please note; lines may be busy at peak times but we will try to answer your call as quickly as we can.

Patient Confidentiality
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care. This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care.

You have a right to know what information we hold about you. If you would like to see your records, please call our Practice/Deputy Manager and this can be organised.

Patient Advice and Liaison Service (PALS)

The patient advice and liaison service (PALS) is an accessible, confidential and free service for people who would like information about the different services provided by the Eastern Wakefield Primary Care Trust and services in Primary Care. Services in Primary Care are services in Doctors surgeries, dentists, opticians and pharmacists.

Patients' Rights

• You have the right to use all the services provided by the practice.
• You have the right to complain if you feel you are not receiving a satisfactory service from the practice.
• You have the right to see a GP of your choice (you may not be able to see the GP of your choice immediately).

Patients' Responsibilities

• To treat all members of staff in a reasonable and courteous manner.
• To make every effort to be punctual for your appointment.
• When you have been given an appointment you are responsible for keeping it or giving adequate notice of cancellation so that the appointment can be made available to someone else.
• If possible please avoid contacting the practice before 10:30am unless you require an URGENT appointment or home visit that day. We have several lines and the call handlers cannot always answer immediately. The call handlers deal with patients in a polite and professional manner and will answer your call as soon as possible, so please be patient.
• To ring before 10:00am if you require the doctor to make a home visit.
• To inform us of any changes of address or contact details.
• Accompany children aged under 14 years when they are consulting a health care professional.
• Ensure your family is fully immunised.

Sickness Certificates

If you are absent from work due to illness for seven days or less, you need to obtain a Form SC2 from your employer or the DSS. If after seven days you are still unwell, you will need to see a doctor who may then issue a medical certificate. Patients who have attended hospital for treatment should acquire if necessary a medical certificate from the doctor seen at the hospital.

Violent or Abusive Patients

The practice adheres to the national guidelines and follows a zero tolerance ruling with regard to abusive or violent patients.

In circumstances where patients are aggressive, violent or abusive towards a member of staff the following action may be taken depending on the severity of the complaint: -

• Patients may be taken off the practice list.
• Patients may be escorted off the premises.
• The police may be contacted.

Your Medical Record

Your medical record is a life-long history of your consultations, illnesses, investigations, prescriptions and other treatments. The Doctor-patient relationship sits at the heart of good general practice and is based on mutual trust and confidence. The story of that relationship over the years is your medical record. We are required by law to allow you access to your medical record. If you wish to see your record, please contact the Practice for an appointment. The Deputy Practice Manager will assist with further queries. A small administration fee will be charged for the procedure.
