Repeat Prescriptions

Repeat prescriptions will be issued to you if you require regular medication. Please allow 48 hours notice for the prescription to be prepared.

To Request a repeat prescription:

bulletBy completing counterfoil on prescription
Tick only the items required on the prescription counterfoil and either leave in the box provided in the waiting room, send by post or hand in at the dispensary.
bulletRequest by phone
You can request your repeat medication between the hours of
 8.30am - 2.00pm (Mon-Fri).
(The dispensary is closed Mon-Fri between the hours of 12:00 – 2:00pm)

Script Location

Please use the space provided to inform the practice where you want your script to go by typing in one of the following options:

bullet Collection from Cohens pharmacy
bullet Collection from Rowland pharmacy
bullet Collect from the surgery
bullet Collect from Ferrybridge surgery
bullet Collect the script only from the surgery

If you do not leave an option the practice will take the same action as your previous medication order.

Medication Home delivery service

A full time Medication delivery service has now commenced and if you are a surgery patient and would like your medication delivering then please follow the process below:

bulletRequest delivery at the point where your prescription is requested from the surgery.
 
bulletPlease request medication in advance. Please do not leave until your medication runs out.

If you are out at the time of delivery the prescription will be available for collection

Medication Reviews

If you are receiving repeat medication you will be required to have a medication review every 6-12 months depending on the medication you are on. This is important, and to ensure that the medication is working correctly (or is not causing a reaction) and for the clinician to assess whether other medication is still appropriate for you.

When your medication review is due, a reminder will show on your prescription counterfoil. To make an appointment, contact reception or dispensary.

Important
If you do not attend a medication review when requested the practice may only produce weekly prescriptions until a medication review has been performed.

Automated Repeat Prescriptions

If you have had a medication review in the last 3 months you now have the opportunity to have your medication automatically prepared for collection/delivery every month for the six months, without having to request the medication.

Important – this service is only available for medication taken regularly where 28 days supply is given and therefore does not include inhalers, eye drops etc. If in doubt please contact the dispensary.

To access this service either:

• Complete the online prescription request providing all the information
• Contact the dispensary by telephone
• In writing with your medication request

You will have to attend the practice to sign a form agreeing to the scheme (a copy will be kept on your notes). This will also provide you with the dates, type of medication and place of collection. From then on there will be medication ready to collect or deliver on those dates.

The scheme can be renewed after that date provided that a medication review has been attended (if required).
