Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out a change of contact details form
• phone or visit the surgery from 8.00am to 6.30pm

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

• Check you are in our catchment area

It is important that you keep your contact details up to date, if you change your mainline number, mobile or email address. Please let us know as soon as possible by completing a PATCHS, calling us on 01977673141 or attending the surgery.

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
